Waiver Services

Headway of Western New York is the Regional Center for two New York State Department of Health programs to help people who have Medicaid stay out of nursing homes or return home from nursing home placement. The Regional Center at Headway is responsible for six western NY counties – Erie, Niagara, Chautauqua, Orleans, Wyoming and Cattaraugus.

All referrals for the counties listed must come to Headway of WNY. The regional center is responsible for the program in this region and must approve all possible applicants to the program before they are able to get services. Referrals can be made by phone or by using the TBI Waiver Program Referral Form, or NHTD Waiver Program Referral Form. Referral forms may be sent by fax or mail. 

The referrals will be reviewed and confirmed. Headway staff will contact the referred to set up an intake. The following intake forms will be reviewed: Form 1 and Form 2. 

Once approved, the service you will receive will be from provider agencies that you are able to choose through this participant driven program.
